This Significant Occurrence in Aviation Report (SOAR) is designed to obtain near real time input from front line operators on issues related to operational safety, effectiveness and efficiency. With encouraged participation from all those involved in your flight operations, it is capable of providing trending information that may go unnoticed otherwise and can greatly improve your risk management.

It is important to note that unlike other "near miss" safety reporting systems, the SOARS form is also designed to collect positive events and occurrences to enable information sharing on what is working as well as allowing management visibility on what needs to be fixed.

What is a Significant Occurrence?

For safety and operational management purposes, a "significant occurrence" is defined as any out of the ordinary event that might have led to compromised mission accomplishment. The report is filled out online and can be submitted either anonymously or with contact information for later follow-up.

Quarterly reports can be provided for analysis. These forms are intended to be used by any and all flight personnel who have witnessed or been a part of an incident or near incident that could have or did have a significant effect on safety. It is intended to be non-punitive, which ensures the integrity of the system and provides a trusted reporting forum that might otherwise not be utilized by flight operations personnel.
